HOME GUIDES & ADVICE

From hospital discharge to senior living: a Buffalo family's checklist

July 2026 CARDINAL CARE ADVISORS

"She can't go home alone." When a Buffalo hospital or rehab says those words, families typically get three to five days to solve a problem they've never faced. Here's the playbook — the same one we run with families on discharge timelines every week.

Day 1 — Get the facts in writing

Day 1–2 — Money and geography, fast

Communities will ask two questions immediately: what's the monthly budget, and who's nearby? Settle a realistic number (our Buffalo cost guide has the ranges) and pick the family radius. A discharge is not the moment to tour eleven places — it's the moment to tour the right two.

Day 2–4 — Tour like it matters, even at speed

Day 3–5 — Decide, document, move

Get the rate, care charges, and move-in date in writing before the transport is booked. Confirm medications transfer with the hospital. And breathe — a good discharge placement can still be re-evaluated at 60 or 90 days once the dust settles; the goal today is safe and sound, not perfect and permanent.
