How to explain an unworkable discharge plan

Bring concerns to the discharge team and ask how they will be addressed. Questions about rights or appeals should go to the hospital's appropriate staff, plan, or qualified advocate.

Keep a factual record of the concern, whom you spoke with, and the response. Ask for the next step in writing when possible.
