What to do when you do not know the care level

Start with the person, then build the plan.

You do not have to choose a label before asking for help. Describe the day: what the person does independently, where assistance is needed, and what arrangements are no longer working.

Ask a qualified clinician or provider which assessment is appropriate. An online quiz can organize a conversation, but it cannot determine whether a community is equipped to provide safe and appropriate support.

Build a plain-language summary for the right professional channel. Keep sensitive details out of public inquiry forms and ask who should receive them.
