Ask for the plan in plain English
- What support will be needed after discharge, and who will assess it?
- Which settings can provide that support?
- What is the expected timing, and what remains unresolved?
- Who is coordinating equipment, transportation, and follow-up?
- Who should the family call if the proposed plan cannot be carried out?
Do not confuse an opening with a fit
A room being available does not confirm that the provider can support the person. Ask the receiving community to complete its review and confirm admission, services, cost, and arrival arrangements directly with the discharge team.
