Information you choose to provide
Our website forms collect your name, email, optional phone number, contact preference, and basic planning answers such as location, relationship, current living situation, and timing. They also record the consent choices you make. Do not include diagnoses, medical records, medications, Social Security numbers, insurance numbers, or other sensitive documents. This public form is not a clinical intake portal.
How we use it
We use request information to respond, organize the requested guidance, and follow up through the channels you select. A guide request is used to provide the requested download. An expansion-notification request is used for that notification, not general newsletters. Checking the text-message box permits request-related texts; it does not enroll you in promotional texts.
Data sharing
Customer data submitted through our forms is not shared with third parties for promotional or marketing purposes. We do not sell family inquiries or distribute your form to a network of communities. We ask before introducing you to a community or other provider.
Mobile opt-in data and consent are not sold or shared with third parties or affiliates for their marketing or promotional purposes. Other sharing described in this policy does not authorize that use of mobile opt-in data.
Service providers process information on our behalf to host the site, receive forms, store requests, and deliver communications. Netlify provides website hosting and form processing. These operational uses are distinct from providing your contact details to a community. We may disclose information when required by law or to protect security and legal rights.
Website measurement
The website may receive technical information such as IP address, browser details, and requested pages through its hosting provider. A local browser preference records whether you accept or decline optional advertising measurement.
If you accept optional measurement, the Meta Pixel may load on general pages and the request confirmation page. Meta can receive visit or event information, device information, and identifiers through its technology. Form answers, names, email addresses, and phone numbers are not included in our measurement events. We do not load this pixel on the intake, care-specific guides, or policy pages. Declining does not prevent you from contacting Cardinal. You can change your choice using Privacy preferences in the footer. Browser privacy signals may also prevent optional measurement.
Changing your choice prevents future optional measurement through this site. It does not retract information already received by a provider. Browser settings can also manage stored cookies.
Messaging program terms and conditions
The Cardinal Care Advisors messaging program provides customer-care texts about a request you make, including questions, scheduling, and follow-up. We send texts only after you explicitly opt in. This website does not collect consent for promotional text campaigns. Messages may be sent from (716) 300-2769.
- Message frequency varies with your request and communication needs. Message and data rates may apply. Ask your wireless provider about your plan.
- Reply STOP to the number that sent the message to unsubscribe. You may receive one confirmation of the opt-out; further program texts stop unless you opt in again.
- Reply HELP for assistance, or contact jake@thecardinalgroup.org.
- Carriers are not liable for delayed or undelivered messages.
- Consent to text messages is optional and is not a condition of receiving our guidance or purchasing anything. You may use email instead.

Retention and security
We retain information as reasonably needed to respond to requests, maintain records, honor consent choices, and meet legal obligations. Access should be limited to people and providers supporting those purposes. No website or communication method can be guaranteed completely secure. Do not submit sensitive health or financial information through these forms.
Your choices and requests
You may ask to access, correct, or delete information you provided, or to stop follow-up, by contacting hello@thecardinalgroup.org. We may need to verify your request and may retain records when required for legal or operational reasons. Applicable privacy rights depend on your circumstances and location.
Children and external services
This website is intended for adults making care decisions and does not knowingly seek personal information from children under 13. Links to CarePair and other websites lead to separate services with their own terms and privacy practices. We do not automatically transfer your website answers to those services.
Changes
We may update this policy as our services change. The effective date above identifies this version. Material changes to consent purposes require an appropriate new choice rather than relying on an earlier opt-in.
